* Service summary Memorial Home Infusion is a pharmacy service provided by Memorial Healthcare System for patients and employees in need of intravenous drug therapy for acute or chronic medical conditions. Memorial Home Infusion provide a wide variety of intravenous drug therapies and works with accredited nursing agencies to educate and teach safe administration of IV drug therapies in the home setting.

Job Description – Clinical Staff Pharmacist (Home Infusion).
Full time clinical pharmacist needed to provide pharmaceutical care to patients discharge home from acute care setting or physician office on intravenous drug therapy. The clinical pharmacist plays an integral role in patient admission process including care coordination with case management and nursing. Fundamental responsibilities of the clinical pharmacist include selection of appropriate drug delivery method for safe administration, oversees sterile compounding preparation, clinical assessment of prescribed drug therapy, product distribution, clinical monitoring, care plan documentation, drug information and patient education.

Experience & Requirements

Experience

Education And Experience:

Graduate from accredited College of Pharmacy
License registered with Florida Board of Pharmacy
Minimum 3 years of working experience as a clinical home infusion or hospital pharmacist
Proficient knowledge of USP and regulatory standards
Ability to independently research and analyze medical literature including drug information, disease states and clinical practice guidelines

Skill Requirements:

Excellent analytical and problem solving abilities
Excellent written and verbal communication; the ability to integrate healthcare team
Customer service focus and patient friendly
Excellent organizational skills and attention to detail
Knowledge of MS Office and other Pharmacy Information Systems ( EPIC, HCN 360, CPR+)
