About This Job

This position is located at MEMORIAL REGIONAL HOSPITAL SOUTH – (Hollywood, FL) – and must be flexible /available to work weekends /holidays on a rotating / scheduled basis.   


Summary:
Coordinates the discharge of patients.

 

Detailed responsibilities:
• Participates in discharge planning including coordinating patient transfers to other departments or facilities, and coordinating community resources. Provides discharge education and resource referrals to patients.
• Secures post acute care placement of patients.
• Communicates with insurance companies/charities to ensure that cost of required care arrangements are covered.
• Performs clerical and customer service functions in support of discharge efforts.
• Participates in the completion of required regulatory documentation for governmental agencies
