We believe that offering a valuable benefits package, in addition to competitive salaries, helps us achieve that goal. Most of all, we believe that Memorial benefits should keep you and your family healthy and living the life you envisioned together.

Health

Medical Plans

Memorial offers two medical plans. Both plans are administered by Community Care Plan.

  • Memorial Managed Care Plan (MMCP)
    This plan is similar to an HMO. You’re not required to meet an annual deductible, and specific co-pays apply when you receive healthcare services that are covered by the plan. However, MMCP does not cover services by providers who are out of its network, except in an emergency or if a service can’t be offered in-network.
  • Memorial Consumer Health Plan (MCHP)
    This plan is similar to a PPO. It includes an annual deductible that you must meet before it pays any benefits to you. It has out-of-network benefits, which means that you can receive care from providers who are out of the plan’s network if you pay a higher deductible and co-pay.

Dental Plans

Memorial offers two dental plans. Both plans are administered by Cigna Dental.

  • Dental HMO
    In this plan, you may only choose from in-network dentists. If you choose an out-of-network dentist, you’ll pay the full cost for his or her services.
  • Dental PPO
    In this plan, you can choose to see either an in-network or out-of-network dentist, but you may pay more out of network.

Vision Plan

Memorial’s vision plans provide affordable, quality vision care nationwide. Through Humana Vision’s provider network, you can obtain comprehensive vision exams, eyeglasses, and contact lenses.

Virtual Doctor’s Visits

If you can’t make it to a doctor’s office or care center, you can use MemorialDOCNow to consult online, via your phone or tablet, with a physician privately and securely – and at no cost for you and your dependents who participate in either MMCP or MCHP.

Flexible Spending Account (FSA)

Memorial offers two types of Flexible Spending Accounts (FSAs) administered by P&A Group.

  • Healthcare FSA
    Set aside pre-tax earnings every pay period, which you can use all year to pay for eligible medical, dental, or vision expenses, as well as prescriptions that aren’t covered or are only partly covered by your insurance. Memorial will automatically withhold your contribution from your paycheck so it doesn’t count as taxable income. You will receive a debit card to use to pay for qualifying purchases. You must use all of your money that you set aside into a FSA, or you lose it.
  • Dependent Care FSA
    Just like your healthcare FSA, a dependent care FSA lets you set aside pre-tax earnings every pay period for child care and other dependent care expenses. Memorial will automatically withhold your contribution from your paycheck so it doesn’t count as taxable income. Remember, just like your healthcare FSA, you must use these funds or lose them.

Vaccines

Memorial’s Employee Health Services offer you yearly and as-needed immunizations, as well as TB screenings, at no cost.

Life Insurance

Memorial offers basic life insurance at the rate of one times your annual pay, plus $10,000, not to exceed $50,000, in term life insurance coverage. This is a free benefit to you – paid entirely by Memorial. You may also buy supplemental life insurance coverage for yourself, your spouse, and dependent child(ren).

Long Term Disability

We provide all benefits eligible employees with a base long term disability plan, free of charge. It typically pays 60% of your salary. You have a number of options available to enhance the benefit.

Wellness

Memorial offers a number of weekly classes and the Memorial Go-Getters group, to help you stay focused on your fitness goals. Our Employee Health office also holds wellness fairs with free blood pressure and cholesterol checks, vision screenings, BMI evaluations, and flu shots. And as our facilities are tobacco-free, we offer nicotine replacement therapy and stop-smoking programs.

Wealth

Memorial 457(b) Plan

We also offer a 457(b) plan through Transamerica, so you can set aside additional money each pay period.

Memorial 401(a) Plan

This is a retirement plan for full-time employees (working 72 hours or more per pay period), who accumulate a total of 1,872 hours in a calendar year, not including overtime. If you qualify for this plan, Memorial will make a contribution equivalent to 2.5 percent of your annual salary. You can invest your contributions into the fund of your choice, and you’ll be 100 percent vested after just five contributions.

Happiness

Pet Insurance

Our pet insurance plans offered through Nationwide provide your dog, cat, or other pet with medical coverage for services performed by any veterinarian – even specialists and emergency providers –anywhere in the US.

Child Care Services

Memorial offers discounted child care services to its employees. To learn more, please call the child care center at (954) 966-4331.

Discounts

Many businesses, educational institutions, and area attractions offer Memorial employees corporate discounts and waivers. You’ll receive periodic emails announcing new opportunities to save money on items such as uniforms, sports tickets, and more. Your Memorial ID badge will also allow you discounts at our on-site cafeterias.

Transportation

As a Memorial employee, you’re eligible for a $25 discount on 12-trip or monthly Tri Rail passes, and for a $10 discount on 31-day passes for Broward County Transit, both payable via convenient payroll deduction. Memorial Employees Federal Credit Union

Memorial Employees Federal Credit Union is located onsite and offers checking and saving account services, plus many benefits such as low-interest rates on loans and credit cards.

Employee Referral Program

Humana Life Solutions

Humana LifeSolutions provides confidential support for marital and family concerns, child and elder care issues, stress and anxiety, occupational and work problems, legal and financial challenges, drug and substance abuse, and more. LifeSolutions can help you develop a plan of action, refer you to community resources, and follow up to find the answers and assistance you need.

Paid Leave

Memorial provides a number of paid leave hours per year, which you can use for personal needs such as vacation, holiday, and short-term illness. The more hours you work, the more hours you accrue.
You can begin using your paid leave after six months of employment, unless your department is closed for a holiday. You can use your paid leave for periods that your department is closed, such as holidays, in advance.

Family and Medical Leave Act (FMLA) Leave

You may be eligible for up to twelve weeks of job-protected leave under the FMLA if you meet specific criteria and experience a qualifying reason for leave.

Military Leave of Absence

You may be eligible for military leave under the Uniformed Services Employment and Re-Employment Rights Act (USERRA) of 1994.

Disability Leave

If you have an illness or accident that causes you to miss more than three days or a maximum of 24 hours of scheduled work time, disability leave can help. You can accrue a maximum of 48 hours per year in your disability account, based on your hours worked (except for overtime) each pay period during the year.

Leave of Absence

Memorial will grant leaves for up to 90 days for medical, personal, or educational reasons.

Bereavement Leave

If you’re a full- or part-time employee, or staff relief, you can use three days of paid bereavement leave at any time for a death in your immediate family – such as your spouse, parent, stepparent, child, stepchild, grandchild, sibling, grandparent, mother-in-law, or father-in-law.

Jury Duty

Jury duty is a civic obligation, so we encourage you to serve if you receive a summons from the court. You’ll be compensated at your normal rate of pay for all scheduled hours of work that you miss during each day of jury service. (You must submit a copy of your summons and attendance to your supervisor.)
