The person in this position will be part of a team responsible for the design, build, testing, and support of the Memorial Healthcare System Population Health Management solution. This individual must have broad knowledge of application integration and workflows common in a health care system across varied specialties and the clinical management of these patients. Primary responsibilities include, but are not limited to building of reports and application workflows to support the implementation of Epic’s population health tools including Healthy Planet, Care Everywhere, Healthy Planet Link, and the Caboodle data warehouse.
